VIDEO AUTHORIZATION AND RELEASE

I hereby grant to Edwin Rutsch, permission to record, use and copyright, for later transmission, publication, or reproduction, my likeness, voice and name, for any lawful purpose, at any time. This includes use in the “Inspiration and TAOLB” Video Project and “Spirit of Inspiration” Video Project. I also grant permission to use any photos, video, audio, drawings and other material that I submit for use in these projects.

I also authorize the use of my name and excerpts for the purpose of promoting and publicizing that program.

I waive any right that I may have to inspect or approve the finished product or the written copy that may be used in conjunction therewith, or the use to which it may be applied.

The undersigned in this release desires to assist in the work of Edwin Rutsch by making the image, likeness, voice and other media of the undersigned available. I have read this agreement before signing and fully understand its contents.
